More than 30 examples of patient safety risks at Stoke Mandeville Hospital have been provided to a charity.
An evidence dossier, collated by Bucks-based disability charity BuDs, will be sent to the Care Quality Commission for it to decide what action to take.
The charity took the unprecedented step after receiving ‘alarming’ reports about care.
Information has been provided by patients, staff and visitors, with many of the incidents relating to the A&E department.
Andrew Clark, BuDS trustees chair, said: “We have been impressed by the high quality of the information that has been given to us.
“It is information relating specifically to patient safety and incidents.
“The vast majority are cases where it appears to be very unacceptable situations that should never happen in NHS hospitals.
“They’re not minor or trivial issues. They’re quite fundamental and in some cases life threatening.”
Bucks Healthcare NHS Trust previously said it was ‘shocked and concerned’ by BuDs taking such action rather than directing people to the hospital’s own complaints procedure.
